| (A) Tuition Amount: |

Tuition for the program is $300.00 USD per day which includes all required study materials, and administration fees. |

| (B) Other Costs: |

Lodging price range is approximately $65.00 upwards, depending on location and season. |

| (C) Payment Schedule: |

The remaining amount to be paid must be submitted with the enrollment form no less than 14 days prior to start date.(amount varies, depending on amount of days you will stay) |

| (D) Late Charges and Cost of Collection: |
| |
WIDH, LLC shall be entitled to recover legal fees and costs of collection should legal action be necessary to recover monies owed. |
|
|
| (E) Refund Policy: |
| |
In accordance with State law, if an applicant is not accepted having made an initial payment, all monies will be refunded. |
|
|
| (F) Three-Day Cancellation: |
| |
An applicant who provides written notice of cancellation within
three days (excluding Saturday, Sunday and federal or state holidays) of signing an
enrollment agreement is entitled to a refund of all monies paid.
If a student cancels after three business days of signing the enrollment agreement, all
monies will be refunded except for the $100.00 registration fee.
More than 40% and less than or equal to 50% At least a 50%.
More than 50% No Refund is required
If a program on a particular start date is cancelled or changed, WIDH, LLC offers a full refund or
the option of choosing another program start date within a year.
The WIDH, LLC is not responsible
for airline tickets, other tickets or payments, or any similar penalties that the student may
incur because of any program date cancellation or change.
Students must notify the Director of WIDH, LLC in writing or by certified mail if they need to cancel
out of a Program. The amount of the refund is determined by the expected last date of
attendance and the notice must be signed and dated by the student.
Book costs are non-refundable after three business days.
The costs associated with prerequisites are separate from those of the tuition. The costs of
prerequisites are not to be part of the refund.
The Institute regrets that it cannot make exceptions to this Refund Policy for any reason
including personal emergency. However, the student has the option to take the Program
within a year with a transfer fee of $50.00 and a re-registration fee of $100.00.
Refunds are mailed within 30 days of the date of notification. |

| (G) Job Placement Disclaimer: |

This institution does not guarantee job placement to graduates upon program/course completion or upon graduation. We do offer assistance, but no guarantees. |

| (H) Holder in Due Course Statement: |
| |
Any holder of this consumer credit contract is subject to all claims and defenses which
the debtor could assert against the seller of goods or services obtained pursuant hereto
or with the proceeds hereof. Recovery hereunder by the debtor shall not exceed
amounts paid by the debtor.

| (I) Procedure for Withdrawal: |

Withdrawal requests must be written and dated. |
|
|
| (J) Dismissal Policy: |
| |
Failure to satisfactorily complete the academic or internship aspects of
the training, non-payment of tuition, unethical behavior, or failure to comply with the
rules as stated in the program catalog and code of ethics will result is dismissal from the
school. In which event, the provisions of Paragraph (E) shall govern the Applicant’s entitlement to refund of tuition. |

| (K) Professional Standards: |
| |
Certification standards vary greatly in different jurisdictions. Our
program meets or exceeds the standards set forth by the International Association for
Colon Hydrotherapy, the largest professional organization overseeing standards in the field of Colon Hydrotherapy. |
|
|
| (L) Notice to Applicant: |
| |
To fulfill the Intestinal Health Institute’s intention of providing high quality
education, the school may from time to time, modify program content or
schedule in order to improve curriculum and/or reflect changes in national standards.
The WIDH website which stands as our digital catalog dated March 26, 2008 is incorporated as part of the Enrollment
Agreement. By completing and submitting this application,
I acknowledge that I have downloaded a copy of the catalog, and understand the terms and conditions thereof. |
